Biasanya untuk Matriks 5x5 dan 4x4, tahap tindakan ada tiga (merah, kuning dan hijau). Saya pernah melihat Matriks 7x7 dengan tahap tindakan yang dari lebih dari tiga. Malah saya diberitahu ada Matriks 9x9 dengan banyak tahap tindakan.

Ini kali pertama saya melihat Matriks 5x5 dengan lima tahap tindakan. Ini bermakna tahap-tahap tindakan lebih dikhususkan. SHO dan majikan lebih yakin untuk mengambil tindakan berdasarkan pencapaian Risk Rating.

Semasa baru bekerja sebagai SHO di kilang tayar awal 90an dahulu, antara tugasan yang diberi oleh majikan saya ialah menterjemahkan dokumen-dokumen Bahasa Inggeris seperti peraturan, garis panduan, papan tanda dan sebagainya.

Antara topik kursus yang saya gemar kendalikan ialah Accident Investigation. Sudah tentu kursus ini dimulakan dengan bab Accident Prevention.

Saya juga menggunapakai beberapa kajian kes sebenar yang saya kendalikan semasa saya bekerja di industri dahulu. Saya kaitkan juga dengan perundangan yang sedia ada berkaitan dengan kemalangan tersebut.

Disamping itu saya menggunakan lukisan grafik computer Acad untuk menerangkan lebih jelas keadaan lokasi kemalangan dan bagaimana kemalangan berlaku (sebelum dan selepas).

Sebagai satu contoh kajian kes seperti di dalam gambar, antara perundangan yang saya gunakan ialah:-
